      Food Vendors that serve food consumed during the fair or pre-packaged food that is consumed onsite will be charged a facility buy-out fee. Buy-out fees are calculated using the linear frontage length of an exhibit booth and are a function of the type of food being distributed. Accepted applicants will be invoiced separately from this form. Food samples must by 2 oz or less.
      Including food in your exhibit may require a Health Permit and associated fee. Details will be emailed to you after acceptance. Prohibited food items to sell or sample include: popcorn, all non-alcoholic beverages, and all alcoholic beverages. All food sampling is required to follow all Tulsa County Health Department requirements.

      Electricity is accessible from communal floor boxes located throughout the facility on 30 foot centers. Power is 120V 20A single phase. Exhibitors must furnish their own extension cords.

      To ensure you have adequate power onsite, it's essential we know in advance the specific amount you need. To find the total amperage you require, find the amperage listed on the back of each piece of equipment you plan to use onsite. Then, add up the amps and round up to the closest amperage below. Please check your equipment, and don’t guess.

      Will your exhibit produce any waste?
      Waste is anything you expect to dispose of during setup, showtime or load out at Maker Faire, such as recyclables, compostables, garbage, reuseable/donatable items, and water.
      Maker Faire Tulsa provides trash bins (not trash dumpsters). All makers are responsible for disposing of their waste into proper disposal bins in a responsible manner. Hazardous waste cannot be disposed of at Maker Faire Tulsa and must leave the exhibit building with the exhibitor. Non-hazardous liquid waste must be disposed of in a Maker Faire Tulsa designated drain.

            Participation Agreement: Release and Consent

            In consideration of participating in activities sponsored by the charitable non-profit corporation Fab Lab Tulsa, Inc. and/or The University of Tulsa (collectively “FLT”) I hereby understand and agree to this Adult Waiver and to the terms hereof as follows: RELEASE OF LIABILTIY: I acknowledge that the activities sponsored by FLT are subject to mishap and even severe injury to participants. I understand and acknowledge that the FLT activities have inherent dangers that no amount of care, caution, instruction, or expertise can eliminate. As such, I release and hold harmless FLT, its board of directors, officers, elected officials, agents and employees from any and all liability, claims, demands or causes of action that minor participant may hereafter have for injuries, damages or treatment of said injuries arising out of participation in any and all activities at FLT, including, but not limited to, losses caused by the negligence of third parties and/or FLT. I further agree that I will not sue or make a claim against FLT for damages or other losses sustained because of any injury or treatment thereof sustained from participation in FLT activities. I also agree to indemnify and hold harmless FLT, its board of directors, officers, elected officials, agents and employees from all claims, judgments and costs including attorney's fees, incurred in connection with any action brought because of participation in activities. MEDIA RELEASE: I hereby certify that I freely and voluntarily grant permission to FLT and/or its designated representative to use my image and likeness, including photo, video and/or audio, for FLT approved promotions and media-related activities. Any photo, video or audio recordings produced because of this agreement becomes the sole property of FLT and may be used by FLT for any purpose, including reproduction and dissemination via various communication channels such as newspapers, sales collateral, website, and social media, without compensation. I hereby release FLT from liabilities arising out of what I might deem misrepresentations by virtue of distortion, optical illusions, or faulty mechanical reproductions. CONSENT TO RENDER FIRST AID / EMERGENCY CARE: In the event that I am injured or ill while participating in FLT activities, I hereby give permission to the FLT (its employees, agents or representatives) to provide first aid and to take the appropriate measures, including contacting the Emergency Medical Service (EMS) system and arranging for transportation to the nearest emergency medical facility. RESEARCH CONSENT: As a participant in FLT activities, participants may be asked to take part in a research study to measure the impact and outcomes of FLT programs. The information from the research study will be used to improve programs at FLT for future participants. Participation in the study is voluntary. If participants choose to take part, they are free to not answer any question or discontinue participation at any time without penalty or loss of benefits to which they are otherwise entitled. There are neither risks nor benefits associated with participation in the study. The anonymous data from the study will be kept private during collection and data analysis. Only approved researchers will have access to records. Results will be reported only in an aggregation of participant responses and the individual will remain confidential. PRIVACY POLICY: FLT is committed to protecting personal information and respecting the privacy of its patrons. The personal information FLT collects is used to deliver, manage, maintain, and improve its public programs and services. To prevent unauthorized access or disclosure of personal information FLT has put in place suitable physical, electronic, and managerial procedures to safeguard and secure the information it collects. FLT does not share, sell, or store personal information. I hereby consent to participate in activities at Fab Lab Tulsa and/or The University of Tulsa as described within this Adult Waiver.
